Osteopractic Physical Therapy is a sub-specialty within physical therapy, and more specifically brands the kind of physical therapy services (rather than simply “physical therapy”) offered so prospective patients and colleagues alike, can identify the appropriate practitioner to most effectively treat their condition.

When you break a bone, you don’t go to your primary care doctor, you look for an orthopedist.  When you have a skin condition, you go to your dermatologist. Likewise, when you have neck and back pain, headaches, tennis elbow, carpal tunnel syndrome, knee osteoarthritis, plantar fasciitis, shoulder impingement, or joint pain etc., it makes sense to look for a specialist within physical therapy, i.e. an Osteopractic Physical Therapist or Osteopractor, not a generalist, that is specifically trained and has advanced post-graduate qualifications to treat neuromusculoskeletal conditions.

Even though the term Osteopractor literally translates to “bone practitioner”, the latest evidence-based practice guidelines for musculoskeletal disorders clearly supports a “multi-modal” approach; therefore, in addition to the joints (i.e. the bones), the treatment of myofascial trigger points, tendons, ligaments and fascia which all connect to the bones is certainly recognized within the osteopractic approach for the most effective patient management. Likewise, a neurosurgeon doesn’t just operate on nerves during surgery, and osteopaths don’t just treat bone diseases.

According to Dr. Dunning, the term osteopractor has nothing to do with the chiropractic or osteopathic professions; that is, the osteopractic concept is firmly focused on the management of neuromusculoskeletal disorders in an evidence-based fashion, not the treatment of other organ systems as the profession of chiropractic has traditionally engaged. Osteopractors do not subscribe to the theory of the “Vertebral Subluxation Complex” as the primary cause of “dis-ease”. Osteopractic Physical Therapists do not diagnose or treat all 10-organ systems as chiropractors are trained and licensed to do, and they do not utilize medicine or surgery as osteopathic physicians are trained and licensed to do. Additionally, spinal manipulation and dry needling are shared procedures between many healthcare professions (i.e. no one profession owns these procedures, e.g. 2012 Supreme Court Ruling: Alabama State Board of Chiropractic vs. James Dunning); however, the philosophy, clinical reasoning, and the conditions treated with these procedures dramatically differs between professions.”

Success Story: Conquering Back Pain and 4,000 Footers!

Thanks to Dr. Greg, my intermittent and sometimes incapacitating back pain has dramatically improved. After assessing my movement patterns, Greg identified deficits and then taught me strengthening and stabilizing exercises that specifically targeted my problem areas. On my very first appointment, he was also able to treat me using dry needling, an Osteopractic technique that targets underlying “stuck” trigger points to improve neuromusculoskeletal pain and movement impairments. The dry needling treatment gave me immediate benefit such that I left the appointment feeling like myself again vs feeling like a 90-year-old when I arrived. Success Story: 95% is not enough!

I sustained a low back injury in the fall of 2020. Having dealt with lingering pain for the better part of 2 years, I have quickly come to understand that the human body is incomparable when it comes to the art of compensation. I don’t have enough pain to stop me from lifting heavy weights and training efficiently, but it is enough to plant a seed of fear in my mind with every rep I pull from the floor. After 6 months of weekly traditional physical therapy with no set diagnosis or noticeable improvement, I tried to accept that 95% was as good as it was going to get, and I would compensate for the rest.

I am a competitive athlete. I lift weights because I love the sport, but more so because I firmly believe in pushing the limits, and breaking the glass ceiling of what weight training is “supposed” to look like as a woman in the industry. With this mindset, 95% just doesn’t cut it…and the result was the exact injury reoccurring in June of 2022.

When I went to Precision Osteopractic Physical Therapy, the first thing Dr. Tae said to me during my consult was “95% is not enough, we are going to get you the 5% back.” That was 6 weeks ago, and together we reached that goal.

Dr. Tae’s approach to physical therapy is extremely comprehensive, his treatment plans are succinct, and he was able to give me an injury diagnosis during my first appointment. However, what impresses me the most is his commitment to an individualized, compassionate, doctor-to-patient experience. After all, healing is non-linear, and the process is unique to the individual; it cannot and should not be generalized. Dr. Tae immediately took a genuine interest not only in who I am as an athlete, but who I am as a human.

The combination of hands-on treatment provided during in-house appointments, the trusting, collaborative therapeutic relationship developed with Dr. Tae, and the rehabilitation exercises incorporated into my daily routine yielded a small fraction of improvement every day. Osteopractors: Your Body’s Best Mechanic

A good automobile mechanic has the ability to accurately diagnose, test, and repair the issue at hand. Differential diagnosis is the process by which a medical practitioner collects data through conversation and functional testing to develop an understanding of a patient's body and symptoms. Osteopractors’ subspecialty training enhances their ability to apply knowledge of anatomy and biomechanics in order to provide targeted intervention using a specialized skill set through which meaningful improvement is quickly realized.

Much like the “check engine light” turning on in a vehicle, pain and dysfunctional movement patterns are an indicator that something is wrong. The root cause may still be unclear. Osteopractic training strengthens the process of differential diagnosis, leading to specificity and full understanding of the problem. Your pain is from your pattern

Your body does not know the right way to move. It is incredibly intelligent and we are able to do incredible movements, complete challenging tasks, and achieve excellent feats without having to individually control each individual muscle in our bodies. We tell our body to run, bike, climb, swim, drive, & sit; it will accomplish the task. But that comes with a caveat, it does not ask permission or ensure that you move in the most efficient or mechanically advantageous way. Your body likes the path of least resistance. 

What does that mean? 

Chronic pain can be simplified into two categories: Structural/mechanical pain and pain from dysfunction. Think of structural pain as a damaged puzzle piece, broken or non functional (i.e. a rotator cuff tear). In the event of a structural issue, our body adapts and attempts to promote function while limiting pain or further damage. In our example of a rotator cuff tear, our body might use muscles like the trapezius to compensate for the damaged rotator cuff tendon. This compensation can result in the second type of pain–pain from dysfunction. 

Just as square tires are not the most efficient shape to move your car, the way your body compensates is less efficient but it gets the job done. This is where pain from dysfunction begins. When our body creates new movement patterns, we wear down different structures as they work double duty for something that is damaged. When this happens, muscles become stiff due to compensation and can become pain generators that reduce our function and our ability to complete our tasks.

The answer:

Damage to a muscle/structure/tendon is not final. With our natural healing process our body can often get close to full function again. Sometimes it needs a nudge in the right direction. With effective intervention and meaningful movement and exercise, we can retrain a dysfunctional pattern, foster an effective healing process, and promote reduced pain and improved function.

Success Story: CrossFit Champion

I’ve played sports and have been very active outside since I was a kid. I did collegiate track and field and shortly afterward found the sport of CrossFit. Outside of the gym I very much enjoy hiking, biking, running, swimming and hunting. I truly do believe movement is medicine for both the mind and body. So I think it’s very important to treat and take care of your body. 

Enter Dr. Greg and Precision Osteopractic. Greg is well versed in many aspects of treatment which I think is very important and makes him a very effective practitioner. I started seeing Dr. Greg at Precision just before the CrossFit Games Quarterfinals. I was trying to deal with some chronic hip and shoulder pain that wasn't really going away with my normal rehab exercises. I didn’t have the luxury of taking time off and knew I needed to seek some expert advice. I booked my first session at Precision Osteopractic right at the RVC.  During my initial evaluation including a thorough history, we discovered that my unresolved hamstring tear from high school was a major contributor to my chronic hip pain. Dr. Greg was able to quickly identify my pain generators and effectively treat them with trigger point dry needling, adjustments, and manual therapy techniques. The dry needling looks painful, but it’s really not, and I was very impressed with how effective it was. How to love your feet so they love you back

Let’s talk about your feet. On Monday morning you rise for the grind & slide on your daily driver shoes which contain off-the-rack orthotics. Perhaps they were recommended for flat feet or high arches. Let’s clarify some misunderstandings about feet, orthotics, and how you can love your feet for the long run.

The feet are astonishingly intricate, just like the hand. The brain has similar amounts of space dedicated to the control of the feet, ankles, and toes as it does for the hands, wrists, and fingers. Both hands and feet are incredibly complex, requiring neuromusculoskeletal coordination to ensure that both static and dynamic posture are effective. Without proper control and strength, injury, dysfunction, and pain can occur.

First, static (still) posture is only one component of structure & function. Many patients mention they have been told at one point or another that they have high arches, flat feet, or another postural anomaly. Fortunately, these variations in resting posture don’t doom the feet to problems. Dynamic posture refers to the changes that occur as we move. When walking, the foot goes through phases and the arch is dynamic, meaning that it changes during various parts of a single step. This dynamic posture is critical for function. The heel first strikes at the beginning of a step, creating rigidity so the foot can absorb the force of our body moving towards the earth. Then, as our weight transfers over that planted foot, it pronates, creating a normal, relative “flattening” of the arch. At the end of the stride, the foot then supinates, or builds an arch again, creating rigidity so that we can push away from the ground. When there is a decrease in the foot’s ability to change its posture throughout a stride, problems can arise.

Fortunately, by working on ACTIVE strength and stability through our feet and ankles in functionally meaningful ways, we can successfully address pain and dysfunction. For example, by weight bearing/standing with forces in multiple planes, we can promote the neuromuscular ability to support the skeleton of our foot. This approach promotes longevity and reduces potential for injury, regardless of footwear or orthotics. Therapy ensures the foot has its own robust internal support, so that shoes which provide extra support are truly giving EXTRA support in addition to what has been cultivated in the muscles and connective tissue of the foot itself.

My journey with Precision Physical Therapy began when I was sixteen years old and suffered from IT Band syndrome. I heard about Precision Physical Therapy through my dad, who had previously gone to Dr. Greg when he was in need of treatment. Despite the many challenges of the 2020 pandemic, it turned out to be a great year for open-water swim training. While there were no official events, as a teacher with the summer off, and nothing else going on (no travel, no weddings, etc.), I was free to swim more than ever before. Inasmuch, I trained for and swam a solo 10k open-water ‘event’ in July. Logging weekly totals over 20,000 yards, combined with plenty of dryland work had me feeling invincible by the time school started. Unfortunately, that feeling came to an abrupt end in mid-September when I suffered an acute injury to my left shoulder while swimming butterfly in the pool, and couldn’t raise my arm past 20˚ the following day.

Are cortisone shots all they’re cracked up to be?
